Enrolling and Changes:
To prevent fraud, direct deposit forms are available by request only (via email firstname.lastname@example.org or in-person from the Payroll or Human Resources Office). A voided check or other verification of Routing and Account numbers must accompany the Direct Deposit form.

Although all employees are encouraged to use direct deposit, there are circumstances in which an employee may need a paper check. To stop an existing direct deposit arrangement, please complete the Direct Deposit Cancellation form.
