Important Health Coverage Tax Documents
This notice is to inform Current and Former Employees, as well as any non-employee who is enrolled in the plan that the Internal Revenue Service (IRS) Form 1095, reporting your offer of health coverage as well as health coverage information for the 2025 tax year, is now available.
How to Request/Access Your Form
- Current Employees are able access an electronic copy through my.albion.edu.
- Login using your Albion credentials.
- After logging in navigate to the Employee Dashboard > Taxes > 1095-C Employer Provided Health Insurance Offer and Coverage Statement Provided the requested information> Display> Printable 1095-C (found at the bottom of the page).
- You may request to receive a printed copy to be sent to you via the process listed below.
- Former or Non-Employees may receive a copy of their statement upon written request.

What is Form 1095?
The Form 1095-C is an IRS tax document provided to employees by the College. The form details health insurance offers and enrollment for the previous calendar year. It is not required when filing a tax return but should be kept for records.
