Hiring Process

Employment Changes

  • End of Employment: Please use this form to report critical information to HR when an employee is leaving employment.
  • Offboarding Procedures: Please use this list of procedures as a list of responsibilities when an employee is leaving employment.

New Hire Evaluation Resources

  • Corrective Action Form: Please use this form to document corrective action regarding a staff member.

Annual Performance Review Resources

  • Online performance evaluation tool – Supervisor Portal
  • As part of the annual review process employees are to complete a Performance Review Self Evaluation and provide a completed copy to their supervisor to discuss the employee’s assessment of their performance along side the supervisor’s assessment.
