Drop/Add and Special Registration

This form is to be used when students wish to Add and/or Drop courses they are not able to Add/Drop on their own. Examples include:
- Adding courses that are full
- Waiving prerequisites
- Adding a course that is a time conflict with another course

This form should also be used to indicate courses that must be dropped in order to add another course. Maximum enrollment is 4.5 Units.

Please provide all necessary information for the courses you wish to ADD. Incomplete forms cannot be processed.

If the course requires a Lab, please include the information for both the Lecture and Lab.

Example: For BIOL 195, Subject = BIOL; NUMBER = 195

It is recommended that you contact the instructor prior to submitting this form.

When you submit this form, a copy will be sent to all email addresses you provided above. Instructors will receive copies of your request if their permission is required to add their course. Instructors will return the form to the Office of the Registrar with their decision, after which any approved changes will be made.
Allow 1-2 Business Days for the requested changes to be made.
