This form is to be used for students who wish to Withdraw from a course after the Drop/Add period has ended. Students who Withdraw will receive a grade of W for the course (this does not affect GPA). Students do not need permission to Withdraw from a course, but are advised to discuss Withdrawing with their advisor prior to completing this form.

If processing this request drops you below 3 Units, you will become ineligible immediately, from the moment the form is processed, to participate in a varsity sport, and we will notify the Athletic Department of the change to maintain NCAA compliance.(Required)
