Physical Forms/Insurance Information

In an effort to provide the best health care for the student-athletes at Albion College, the Athletic Training Department must have a physical examination, health insurance information and authorization, assumption of risk, and student-athlete health history questionnaire forms completed and on file before the student-athlete is permitted to practice. The forms can be returned to the following:
