In order to become a fully enrolled student and to allow the college to make the best preparations for your arrival, we ask that you submit the requested personal information listed in the grid below. Some of the information is required some if the information is optional. Please check the listings carefully to determine what pieces of information are best submitted for you personally.

Keep in mind the required information is extremely important for college departments and by obtaining this information will aid in a smooth transition to Albion College. It is most helpful to have all of the information submitted a week prior to your arrival at SOAR program. However if that is not possible, you can bring the completed information with you to the SOAR Program. The earlier we receive your information, the better we as an institution are able to be prepare for your arrival.

Forms
This site offers two options for completing your forms.

Interactive forms: You can also fill out and submit your forms online. These forms are completely secure so that any information submitted will remain confidential.

PDF forms: You can download PDF versions of the forms, print them on your home printer, fill them out by hand, and bring them with you to the SOAR program:
