The following worksheet has been prepared to assist you in keeping track of how you have fulfilled various pre-health profession school application expectations.  The use of 3 lines for various work/volunteer experiences is not meant to convey that 3 experiences are required (or that you are limited to 3 experiences); however, having a broad range of experiences can help you make sure you are making he right career decision.   (Feel free to copy this page to include in your digital portfolio or print it to keep with your other advising materials.)
