As required by the Family and Medical Leave Act (FMLA), the College
will provide covered employees up to 12 weeks of unpaid job-protected
leave for certain family and medical reasons. Employees who have worked
for the College for at least 12 months and for 1,250 hours over the
previous 12 months of employment are eligible.

Except in cases of child placement after adoption or foster care, a
Certification of Health Care Provider form is also required, please
print the appropriate form and have it completed by the health care
provider and return with your Medical Leave request form.
